This case was particularly challenging. The patient had significant scarring on his upper lip from multiple cleft palate surgeries, which had prevented any hair growth. I promised his mother we could restore his moustache and beard, and this video shows both my explanation of the complex procedure and the patient's own words about his experience five months later.
Here you can see a direct before and after comparison for the same patient. We successfully filled in the patchy areas on his lower beard and created a full, natural-looking moustache over the difficult scar tissue on his upper lip using a targeted FUE transplant.

Implanting hair into scar tissue is different from standard beard restoration. Scar tissue often has a different blood supply and density compared to healthy skin, so the angle and depth of each follicle must be precise. I use high-magnification mapping to ensure the transplanted hairs match your natural facial hair direction, blending them seamlessly with the surrounding skin for a permanent, undetectable result.
Understanding Scar Tissue Reconstruction
When a patient comes to me with scars from cleft lip repairs or previous injuries, the challenge is not just filling the gap. Scar tissue is fibrous and often lacks the elasticity and vascularity of healthy skin. This makes it a difficult surface to work with, which is why many clinics might turn these cases away.
My Approach to Scar Coverage
I focus on a few key technical areas to ensure success:
- Angle Matching: Facial hair does not grow straight out. I meticulously map the natural direction of the beard or moustache and create recipient slits that match these angles exactly. This prevents the 'pluggy' or artificial look often seen in failed surgeries.
- Micro-punches: I use 0.7mm to 0.8mm micro-punches for extraction and implantation. This minimizes trauma to the tissue and ensures the density looks natural.
- Tissue Prep: Before placing grafts, I ensure the scar tissue is prepped properly. In some cases, we use PRP (Platelet Rich Plasma) therapy to improve the blood supply to the scarred area, which significantly boosts graft survival rates.
What to Expect
If you have a scar you want to cover, we start with a consultation to assess the health of the tissue. We do not rush this. The goal is to provide a result that gives you confidence, not just a patch-up job. Recovery generally follows the same timeline as a standard beard transplant, with final results visible around the 9-12 month mark.
