Most deep acne scars are tethered to the underlying tissue by fibrous bands, which is why serums and creams simply don't reach them. My approach starts by physically releasing these bands through subcision, often followed by MNRF or CO2 laser resurfacing to stimulate fresh collagen. We don't guess; we first assess your scar depth and pigmentation risk to ensure we don't accidentally cause further skin damage.
Beyond the Surface: Why Scars Won't Fade
Many patients come to my Koramangala clinic having spent thousands on 'miracle' serums that promise to erase years of acne marks. The hard truth is that deep boxcar or rolling scars are caused by lost tissue and fibrous tethering. Topical products work on the epidermis, but scars live in the dermis. To fix them, we have to go deeper.
My Treatment Protocol
I focus on structural repair using a combination of these methods:
- Subcision: This is the non-negotiable first step for many tethered scars. I use a specialized needle to break the fibrous bands pulling the skin down, allowing it to lift and level out.
- Microneedling RF (MNRF): Unlike standard microneedling, this combines mechanical injury with thermal radiofrequency energy. It’s effective for deep remodeling but, more importantly, it carries a lower risk of post-inflammatory hyperpigmentation compared to aggressive CO2 lasers, making it safer for many of my Indian patients.
- CO2 Fractional Laser: For skin resurfacing, this is highly effective, though it requires a strict post-care routine. I assess your skin type before recommending this to ensure we avoid complications.
