Our Gynecomastia Surgical Technique
We do not just remove fat. We use a dual-action protocol combining precision liposuction and gland excision to fix gynecomastia permanently. Here is how we ensure a natural, masculine chest contour with minimal scarring.
This video breaks down our surgical process. You can see the liposuction cannula being used to remove fat, which is a key first step in contouring the chest. The immediate post-op view shows the result of this combined technique.
Here, we show the liposuction machine in action as it suctions out the emulsified fat. This step is crucial for sculpting the chest and removing the fatty component of gynecomastia, leading to the flat result seen post-op.
A critical part of the surgery is the removal of the glandular tissue, which you can see in this video. Excising the gland is what makes the results permanent, as this tissue cannot grow back.
About Our Surgical Technique
Most gynecomastia cases require a dual-action approach because a flat chest needs more than just fat removal. We combine VASER liposuction to contour the chest with a precise periareolar incision to physically remove the hard glandular tissue. This combination is essential because glandular tissue, unlike fat, will not respond to diet or exercise; removing it manually is the only way to ensure the chest does not remain puffy or uneven.
Many patients ask why we do not just use liposuction. The reality is that gynecomastia often involves both excess fat and firm, fibrotic glandular tissue. If we only treat the fat, the gland remains, and the chest often retains a puffy, unnatural shape.
Our Surgical Process
- Assessment: We first identify if your case is fat-dominant, gland-dominant, or a mix of both. This determines if you need standard excision or advanced skin redraping.
- Anesthesia: Most procedures are done under IV sedation, meaning you are comfortable, breathing on your own, and usually discharged on the same day.
- The Technique: We make a small 2-3cm incision along the lower border of the areola. This location is chosen because the scar naturally blends into the border between the areola and the skin, making it very difficult to spot once healed.
- Extraction: Through this opening, we carefully dissect and remove the hard gland. We then use liposuction to smooth out the transition between the chest and the armpit, ensuring no 'side bulges' remain.
Recovery and Expectations
We are transparent about the recovery process. You will need to wear a pressure garment for a few weeks to help the skin retract and the chest settle into its new shape. While the results are visible immediately, full refinement happens over 3-6 months as residual swelling fades. We do not promise that scars will disappear completely, but by placing the incision strategically at the areola border, we ensure they are discreet and fade significantly over time.
